Knowing someone with a severe allergy (whether it be to food, dye, scents, animals, medications, etc.) should be something taken very seriously. Especially if it is a student in our classroom/school building. Being aware and knowledgeable of these students with allergies is huge in helping all students stay safe. A reaction is serious and very scary.
My school’s nurse provides a confidential list at the beginning of the year to all teachers informing us of important student medical issues – including severe allergies.
Having taught students with with severe allergies to scents and peanuts, I wanted to do more about this. I created classroom Allergy Awareness Posters that prohibited both of these items from coming into my classroom, by anyone who was entering it.
However, I wanted to also create specific confidential lists for these students with severe allergies. These Allergy Awareness Lists are perfect for classroom use, to leave for a substitute, to keep in the main office, to have during cafeteria duties, to have on playground duties, for bus drivers, and for field trips.
A confidential cover is included for each list. This is a perfect way to keep track of the student with the severe allergy, grade, emergency contacts, and action plan.
Due to the basic/easy to use format, these lists are not editable.
