Preparing for an IEP report or any other evaluation report can be quite stressful and incredibly time consuming. Although I can’t make it easier for you – trust me I wish I could 🙂 , I can at least provide you with a little bit of help when having to ask for nurse input!
In my first few years of teaching, I realized that asking for input was at times difficult. Nurses are swamped with paper work, students being ill, conducting physicals, etc. Sometimes I felt bad sliding an input form into my school nurse’s mailbox, knowing she probably had piles of paper work sitting on her desk. I did, however, need some information for the parents to see, while also adding their own additional comments. After talking to my school nurse this year, she LOVED the idea of me sending her a Google Form to her e-mail So, I turned my paper input form into a Google Form and “prettied up” my original Nurse Input Questionnaire to now share with all of you!

▻ Directions for using the Google Form
Nurses are able to provide input on student health records, medications taken at school, side effects of medication, screening results, reasons the student might frequent the nurses office, and so on.
If you are new to using Google Forms I have provided easy to follow Google Form instructions. The time it takes to create and set-up a Google Form (and it’s collection spreadsheet) I have done for you! …because let’s be real, as teachers we need all the extra time we can get.
