I’m not quite convinced that many high school students know exactly what they want to do after high school graduation, let alone students of the middle school age. Gathering IEP transition information for a student just turning 14, or even of actual transition age, can be difficult. Luckily, this survey helps with just that!
This Student Transition Input Questionnaire is an IEP transition survey – for students of transition age. It is an informal survey/input questionnaire addressing interests, preferences, strengths, and needs – for students who are older than 14, who have just turned 14, or who will be turning 14 within the duration of their current IEP.
There are many areas of focus when working towards transition. The areas included in this updated resource include: Future Post Secondary Education and Training, Future Job Training, Future Independent Living, Future Community Participation, Future Recreation and Leisure, and Extra Support/Outside Agencies that might be needed. This resource was designed to be used as one whole questionnaire, or broken down into the area(s) that you and your student are focusing on most.
This is an updated resource. However, you will be receiving both the original resource file AND the updated resource file that includes a Google Form.

Students are able to provide their input on classes and subjects they do well in, classes and subjects they need more help with, help they like receiving in their classes, their interests, activities they’re involved in, their jobs/work experience, and so much more. Some parts of the PDF questionnaire require students to simply circle appropriate responses, while other parts allow the students to write their response.
If you are new to using Google Forms I have provided easy to follow Google Form instructions. The time it takes to create and set-up a Google Form (and it’s collection spreadsheet) I have done for you! …because let’s be real, as teachers we need all the extra time we can get.
