Preparing for an IEP report or any other evaluation report can be quite stressful and incredibly time consuming. Although I can’t make it easier for you – trust me I wish I could :), I can at least provide you with a little bit of help when having to ask for parent input!
In my first few years of teaching, I realized that asking for input from my students’ parents/guardians was at times difficult. Sometimes I would get too much information, and other times I wouldn’t get enough at all. I created a parent input questionnaire to send home to parents/guardians at the beginning of the year, or even with the IEP invitation. AND – as of lately, my students’ parents have been asking if they can provide input via e-mail. Of course I was alright with this, so I turned my questionnaire paper copy into a Google Form that I can share!

TWO Parent Input Form Options!
Option 1 (basic questions):

Parents are able to provide input for their child’s strengths, weaknesses, learning styles, behaviors, talents/hobbies, etc. Option 1 requires the parents/guardians to write their answers, whereas Option 2 provides parents/guardians with options to answer with.
If you are new to using Google Forms I have provided easy to follow Google Form instructions. The time is takes to create and set-up a Google Form (and it’s collection spreadsheet) I have done for you! …because let’s be real, as teachers we need all the extra time we can get.
