Out of ten years of teaching special education, this is a form I have been using for the past eight years to provide regular education teachers with as a brief look into their students’ with IEPs information – whether it be the first day of school, after an IEP has been updated, or if a new student has been placed.
I complete this form for each student and attach it to their IEP accommodation page(s), printed straight from their IEPs. This way the regular education teacher will have a better knowledge of their IEP student, come first day of instructing them.
Another benefit – regular education teachers will always be provided with a confidential desk copy of important information they need to know. IEPs are always available for regular education teachers to read, but not always at their convenience.

*** Do you already own one of my IEP binders? This “Get to Know Me” information sheet mimics the IEP information sheets found in each one of my binders. The only difference with this product is that it is geared more towards the regular education teacher, rather than myself as the special education teacher, as it has more concise at a glance information, a student schedule sheet, and a list to write student accommodations – if you choose to not attach them like I do.
