Easily receive IEP input, just by sharing a Google Form. Using Google Forms to collect student input has been incredibly easy and most efficient. Students love that they are able to complete this on their own time. I love it because I either send the form as it is to the student’s email, or tweak it a bit for my students’ needs and then send it. Don’t use Google? Each IEP input form comes as a print & go PDF, too!

Description

Once students turn 14 they are invited to their IEP meeting and take on more of an active/vocal role. They also start planning for their life after high school – otherwise known as transition.

However, when I was an elementary learning support teacher, I felt like my students should have a voice in their IEP planning process, as well as start thinking about what they want to do/become when they are older. Yes, I did this even with third graders. Since my time as a learning support elementary teacher (then moving to middle school learning support teacher and to moving to my current high school learning support teacher position) I have had the chance to create different levels of input forms, in order to have my students’ input in their IEP planning.

These forms come in two options that range from being general – in the elementary form – to being more planning for the future specific – in the form for those students approaching the age of 14.

These input forms come as a print and go option, as well as a Google Form option. I have even provided a guide on how I use these forms with my students.

Students are able to provide their input on classes and subjects they do well in, classes and subjects they need more help with, help they like receiving in their classes, their interests, activities they’re involved in, and what they imagine for their future. Some parts of the questionnaire require students to simply circle appropriate responses, while other parts allow the students to write their responses.

If you are new to using Google Forms I have provided easy to follow Google Form instructions. The time it takes to create and set-up a Google Form (and it’s collection spreadsheet) I have done for you! …because let’s be real, as teachers we need all the extra time we can get.
