Transition Tracking Sheets are essentially little booklets that follow each student starting in 7th grade and up until their 12th grade year. You can store each student’s transition tracking sheets individually in a binder, or store all of your students’ tracking sheets together in a binder. These sheets have been a huge help in discussing student transistion accomplishments needs during a student’s IEP meeting.

Transition is a HUGE part of a student’s IEP planning – especially once he or she turns 14.

As a high school special education teacher, I have always mapped out a transition plan for my students in grades 9 – 12 to follow. This plan made sure I was doing all that I could, during each grade level the student was in, to collect important transition information and to make sure I was meeting each student’s transition needs.

Over the past two years, I have been working with one of the middle school special education teachers I work with to help her start tracking transition information/needs the same way I was in the high school. I cannot stress how much of a HUGE help this has been – to both the middle school special education teacher and myself. We had a an actual plan in our hands to follow, to make sure each student was receiving what they needed (yes, we could have checked their previous IEP transition section or looked in their personal record – but that searching can take up too much time).

This plan we’ve been using are my Transition Tracking Sheets. Transition Tracking Sheets are essentially little booklets that follow each student starting in 7th grade and up until their 12th grade year. You can store each student’s transition tracking sheets individually in a binder, or store all of your students’ tracking sheets together in a binder.

What do the Transition Tracking Sheets look like?

Each sheet is a different grade. There are tabs on the sides of the sheets that indicate the grade level. All grade levels include five different areas of transition planning to track – (1)interest/preference assessments, (2)academic assessments, (3)post secondary education activities, (4)employment activities, and (5)independent living activities. You are able to list each assessment/activity your student will be completing during that grade level, the person(s) responsible for implementing the assessment/activity, and the date the assessment/activity was completed.

What will you be receiving in this download?

There are two options of use provided.

(1) A PDF secured file has been included. This PDF is a generic version of the exact same tracking sheets I use. However, I had to make them generic because all schools, districts, and states call their assessments/activities something different. You can either use this in your classroom or as a guide to help you edit your own.

(2) A PowerPoint version that will allow you to edit the charts. This means you can type directly into the charts, so you can make it geared to what you do in your classroom. The titles on the tops of the pages and the tabs are not editable.

I have also included an assembly/how to use guide.

There are also three cover options available. Chose a cover based on how you store your students’ tracking sheets.

(3) An interactive PDF that is secured due to font copyright. This interactive PDF is a generic version of the exact same tracking sheets I use. However, I had to make them generic because all schools, districts, and states call their assessments/activities something different. You can either use this in your classroom or as a guide to help you edit your own.

(4) An interactive PDF that is secured due to font copyright. This interactive PDF has NO text in the charts allowing you to use your device and write in the assessments/activities your students complete.

(5) A link to Google Slides. You can type directly into the charts I have provided in Google Slides, or delete my charts and insert text boxes. The title fonts cannot be edited due to the font copyright.

I have also included a “how to” guide.
